5 Dead in Fatal Head-On Crash on Highway 101 in Spartanburg

Emergency response on rural South Carolina highway after fatal crash on SC-101 in Spartanburg County

A fatal head-on collision on SC Highway 101 in Spartanburg County killed five people in the early hours of Friday morning, including an off-duty sergeant who was returning home from vacation with her spouse, state troopers and county officials confirmed.

Troopers with SCHP said a 2025 Ford SUV and a 2016 Toyota SUV were traveling in opposite directions on SC-101 near Woodruff when the two vehicles struck each other head-on at approximately 1:00 a.m. Five of the six occupants died; one was hospitalized.

The Spartanburg County Coroner’s Office released four victim identities. Deborah Sue Wyatt, 50, of Duncan, was a Ford passenger pronounced dead at the scene. Max Douglas Wyatt Jr., 46, also of Duncan, drove the Ford and was pronounced dead at Spartanburg Regional Medical Center at 4:06 a.m. Both Ford occupants died in the collision.

Three rode in the Toyota. Sergeant Ashley Munoz, 35, of Woodruff, a city officer, was pronounced dead at the scene alongside Maria Del Rosario Munoz, 62, of Greenville. A fifth victim, the third Toyota occupant, had not been publicly identified as of Friday pending family notification.

City officials confirmed Sergeant Ashley Munoz had served the department for 11 years, achieving sergeant rank in 2023. Her spouse, Sergeant Diana Munoz, also a city officer, was also in the Toyota. The two were returning from vacation and share two small children.

Sergeant Diana Munoz survived with critical injuries and was airlifted to a local hospital. She remained in critical condition Friday. Both officers had served the department for over a decade.

Investigators say the crash remains under active investigation. No charges or fault determinations had been announced as of Friday afternoon. Trinity Fire Department crews responded alongside troopers.

City officials opened a public memorial at 204 Halton Road for Sergeant Ashley Munoz on Friday. The coroner’s office is withholding the fifth victim’s name until next-of-kin notifications are complete.

SC-101 is a rural two-lane route through the Upstate between Woodruff and Greer. Nine local and regional outlets confirmed the five-fatality count, with the coroner’s office serving as the primary identifying authority for names and pronouncement times.
